Are you an experienced registered nurse (BScN or diploma) looking for a different kind of practice environment, and comfortable practising both independently and as part of a team? This could be what you’ve been looking for.   As an integral part of our Rapid Response Nursing (RRN) team, you will work with medically complex children and/or frail adults and seniors with medically complex needs and/or high-risk characteristics such as congestive heart failure, to ensure a smooth transition from acute care to home care. You will achieve this in two ways: by connecting with primary care and by providing hands-on rapid response home care.   In fact, you’ll make the first in-home nursing visit to high-needs patients, shortly after their discharge from hospital, during which you’ll confirm the hospital discharge care plan, emphasize the importance of primary care to avoid re-hospitalization, and reconcile medications. Reporting to the Home and Community Care Manager, this self-directed position functions in an advanced nursing role within a collaborative practice model.  The Nurse Practitioner (NP) may: - Support homebound individuals receiving the services of Home and Community Care Support Services (HCCSS) North West who do not have access to a clinician who completes home visits.  - Support patients who may or may not have access to a primary care practitioner but require a Nurse Practitioner to support a palliative approach to care and advanced pain and symptom management. - Provide short term bridging for HCCSS patients who are vulnerable to admission/readmission to hospital with a clear plan to discharge the NP service.  The NP acts as a key resource to all HCCSS and service provider staff regarding palliative care and chronic health conditions.    Demonstrating clinical leadership in a collaborative practice with all care providers including physicians, community care coordinators, community service provider organizations and other community partners, the Nurse Practitioner ensures that patients who would benefit from a palliative approach to care receive appropriate and seamless symptom management in the community setting.  This support can occur throughout the end-of-life trajectory and is not limited to a specific diagnosis. We need caring, motivated people who are driven to help others and make difference in their community, to join our multi-disciplinary team.   POSITION SUMMARY: The Registered Nurse (RN) or Registered Practical Nurse (RPN) – Community Transition assists with hospital transition home for Alternate Level of Care (ALC) patients with an imminent long-term care destination and other high-risk patients with similar needs in community who are long-term care waitlisted, meet eligibility criteria, and are at risk of requiring hospitalization and becoming ALC designated. The Community Transition Nurse (RN/RPN) provides individualized patient care through a holistic and collaborative approach, working closely with the High Intensity Supports at Home (HISH) team, patient/family and primary care provider to develop a unique plan of care specific to their needs.   NATURE & SCOPE: - Assists with hospital discharge and/or environmental transition of high-risk seniors, ensuring the patient is seen in home within 24-48 hours of being admitted to the HISH Program. The RPN will consult with the RN or assigned Care Coordinator when support is required for complex discharges or transitions.  - Coordinates with HISH or assigned Care Coordinator in the development of care plan necessary to provide patient with wrap-around, holistic nursing support to remain safely at home while waiting for LTC placement. - Provides ongoing, short-term support to patients’ through education and coaching, clinical assessment, assistance with medication management, and communication with primary care to reduce hospital admission/readmission. This may include daily visits to ensure patients’ needs are being met. The RPN will work in consultation with the RN or assigned Care Coordinator on complex patient files or issues outside of the RPN’s scope of practice. - Provides in-home nursing visits and manages patients’ care needs as presented, which may also include support with activities of daily living and instrumental activities of daily living (ADL/IADL). - Collaborates closely with the care team to ensure consistent, aligned communication; participates in weekly huddles as required. - In collaboration with the care team, works with patient/SDM and family members/caregivers and primary care provider to develop a transition plan that supports the unique needs of the patient.   QUALIFICATIONS: - Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario; - Minimum 3-5 years clinical experience working as a Nurse (RN or RPN); - Recent and relevant experience, within the last 2-3 years, providing direct, hands-on nursing care; - Certification as a Gerontological Nurse (GNC) or Gerontology Practical Nurse (GPNC) an asset; - Superior clinical assessment skills and care planning skills; - Strong collaboration skills and ability to provide quality clinical service delivery in conjunction with patients and other stakeholders; - Ability to work independently; - Strong critical thinking and problem solving skills; - Effective interpersonal and communications skills; - Ability to provide health coaching and disease-specific education to patients; - Advanced oral and written proficiency in English and French is essential; - Must have valid driver’s license and access to a vehicle; travel to patient homes will be required; - Valid Emergency First Aid Certification and CPR.   Less qualified candidates may be considered.  However, candidates who meet all requirements will be given priority for an interview.   Home and Community Care Support Services North East is an equal opportunity employer. The Nurse Practitioner will adhere to the effective delivery of Nurse Practitioner services for the Palliative Care Consultation Team’s patients, and community patients unable to access care through a family physician, clinic, FHT, CHC, etc. upon referral from Home and Community Care Support Services ESC and in accordance with professional college standards, Ministry Guidelines and ESC policies.  The objective of the services delivered by the Nurse Practitioner is to diagnose, treat, evaluate, consult and manage complex pain and symptom and associated chronic disease management issues during the patient’s palliative trajectory, in order to delay/prevent institutionalization and facilitate reintegration into the community. These services will be available across various care sectors as required including home, hospital, retirement homes and long term care.  The Nurse Practitioner, as a member of the Palliative Care Consultation Team (PCCT), will work collaboratively with Hospice and be responsible for reviewing and completing assessments to assist in providing recommendations and consultative guidance to support challenging palliative symptom management and diagnoses. Travel for this position is required between all sites of the Home and Community Care Support Services Erie St. Clair and there is the potential that the Nurse Practitioner may work out of the Hospice sites.   What must you have? - Registered Nurse, Nurse Practitioner (Current registration with the College of Nurses) - Working knowledge of community resources and roles of health care professionals - Good knowledge of ESC priorities, policies, practices and service standards - Good knowledge and understanding of legislation governing health care activities - Knowledge of direct care/care coordination models used in community health care organizations - Specialized education required in palliative pain and symptom management including a minimum of Fundamentals and CAPCE.
